Every year the NYCDOE requires families to complete their Family Inquiry Form.
Title I funds allow us to provide programs like:
- complimentary uniform
- after school tutoring
- saturday academic support
- after school clubs
- grade wide trips
- schoolwide activities
- discounted SAT Prep
- college Fee Waivers
Students who do not return the NYCDOE mandatory completed lunch are not invited to participate in any of these activities.
*Submit only one form for all children in the household attending schools in grades 3K through High School.
️Completion of these forms can secure much needed Title 1 funding for our school, so it is imperative that we have one completed for every student.️

Please make sure a lunch form is completed ASAP. We need your assistance. The deadline is Friday, Oct 31st. If submitted after that date, then it does not count toward Title 1 funding.
